Park View Surgery is aware contractually, residents living within our boundary with an existing registration at a local Practice can request to change.
In December 2017 with the increasing demand on our services with winter pressures and the pressures in Secondary Care, the Partners' at Park View Surgery agreed to temporarily halt the transfer of patients between Practices. This difficult decision was taken to help us maintain the standard of care we work hard to deliver at Park View Surgery. The ever increasing demand, lack of resources and complexity of needs is creating pressures in General Practice that, if not carefully managed, creates workload that is unsustainable.
Anyone wishing to register with our Practice will not be refused an application on grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition. The Partners' at Park View Surgery feel at this time, they have reasonable grounds to refuse an application to join our list, if the requester, living within our boundary, has a GP locally.

Patients must be registered with us before they can make an appointment.
Registered patients may consult with any GP subject to their availability. Patients may go to either Milnthorpe or Carnforth surgery.
Registering in person
To register with the Practice either complete the appropriate part of your NHS Medical Card or complete a form available from the surgery and return to us with photographic ID. Please ensure we have your correct date of birth. You may print off forms to complete at home, bring them in with you on your first visit to the Practice.
New Patient Questionnaire

Patients registering with us must also fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Click on the link to open the form Online medical questionnaire for new patients
When you visit the surgery for the first time you will be asked to sign the pre-registration form and medical questionnaire to confirm that the details are correct. You may then make an appointment.
When you have completed all of the details, click on the 'Send' button to mail your forms to us. By sending the forms you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect. Please note that the mail box which forms are sent to cannot be monitored 24/7. Please be aware that the form may not be seen promptly if the recipient is on leave. We recommend that you follow up with a phone call to confirm receipt.
GP2GP patient information leaflet
The Health and Social Care Information Centre has produced a GP2GP information leaflet for practices to distribute to their newly registered patients. What if I need to see a GP when I'm away from home?
You may register with us as a temporary resident, either short stay (under 15 days), or long stay (over 15 days but under 3 months).
As a temporary resident we do not have access to your medical record to assist with your consultation. For your safety you must complete this form Temporary residents info sheet and a blue form which will be provided on arrival. Give these to the receptionist who will then complete your temporary registration in readiness for your consultation. If repeat medication is required patients should bring this with them to the first consultation.
